Applying for an Indian passport at BLS International USA - in lieu of a damaged one
Hi Everyone!
I recently applied for a duplicate passport since my passport got damaged. I had a lot of trouble understanding the document requirements on BLS website, specially regarding the number of photos and nationality verification forms required for the same. There was all sorts of confusing information everywhere. So today that I have submitted my application successfully, I thought I should share my experiences and help others. That's when the idea of blogging came to my mind and here comes my first ever post!

Update (10/14/13): I received my new passport today after 2 weeks of submission!
Online application form
Click here to go to the online NRI passport application page.- It is very important to select the correct Indian mission. For me it was USA-NEW YORK CGI. Click here to find out your correct Indian mission in USA.
- Visible Mark - If you don't have any, write NONE.
- Address to be printed on passport - I gave my US address since I did not want to get into the hassle of procuring my Indian address proofs and getting them notarized. However, based on the experiences of our readers, it looks like if you want the exact same Indian address printed on your new passport as you have on your old one, it is OK and no proofs are needed. Otherwise BLS would only print US address on the new passport.
- Other address - Here I entered my Indian address. No proofs needed.
- Phone No - I gave my US cell number.
- Mobile No - I gave my parent's Indian cell number.
- Family Information - The names should be entered exactly as in your old passport.
- Upload Image - I would prefer not to since you anyways have to paste your hard copy photos to many other forms.
- Generate PDF - Click on this button and save/print your application.
Once you print your application form, fill out all the remaining fields -
- Specimen signature - Make sure you sign exactly as you had signed in your old passport otherwise they might ask you to send a notarized self written affidavit on plain paper for change in signature.
- Color photographs - 2 photos are needed.. one has to be pasted on the first page, the other has to be stapled at the end of the last page. For photo specification, click here or here.
- Amount paid - I applied for a jumbo passport so the fee was $191.20. To know how much you'd pay, click here. I had a money order made through USPS (they charged $1.20). You may go to Walmart too to get that for a lower fee.
- For delivery by mail - Enter 0 here since you need to either collect your passport in person or send a prepaid return envelope along with your application.
- When did applicant first leave India - I filled in the date I first flew from India on the current visa.
- How long has applicant continuously resided abroad - This should be the difference in the date you are filling your application form and what you filled in the field which said 'When was he/she in India last?'
- Present Emigration Status - This needs to be whatever you have in your current passport. I was not a professional degree holder when I applied for my first passport so I have ECR written on it. For passports issued in or after Jan 2007, no notation in the passport means ECNR. Documentary evidence is required for ECNR though I've read of some people who were told by BLS that this evidence was not required.
- For all yes/no questions, do write Yes or No (as applicable). For other questions which are not applicable to you, write NA.
- Name and address of two relatives/friends - US based.
- Briefly state circumstances of loss/theft/damage of passport on a plain paper - I didn't need to write anything for damaged passport.
- Declaration - This needs to be notarized (since I was applying under damaged passport). For notarization, I went to UPS who do it for $5 per document (yes, pretty expensive, specially since you'd have to notarize a lot of things). Some banks notarize their customer documents free of cost, mine didn't.
- Specimen signature - You need to sign in both boxes (again, same signature as in old passport!!).
Nationality Verification Form
Click here to download the required nationality verification form. You need 8 copies, each with a photo. That makes a total of 10 photographs for this complete application.
- Place of birth - It asks for Village and District. I just filled the Indian city and state instead.
- Date of leaving India - This should be same as what you filled in 'When did applicant first leave India' in your application form.
- Period of stay abroad - This should the difference between the date you are filling this entry and the date you entered just above as 'Date of leaving India'.
- Date of loss of passport - Leave it as blank or write NA since we are applying under damaged passport.
- Name and address of travel agent involved in sending the individual abroad - If it is your company which sent you (like mine), you may write your company's name and Indian address. Otherwise, write the travel agent's details.
BLS Order Form
Click here to fill out the BLS order form. This is a must have for both postal and walk-in applications. In case you plan to drop your application at the BLS office and pickup your passport when ready, you do not need any airway bill numbers (tracking numbers). Otherwise, please make sure you generate prepaid labels as needed before filling out this form. I would suggest creating a UPS account for doing this online. FedEx may also be used. I don't think USPS can be used for return labels since they do not provide tracking numbers until you are actually shipping. Creating an online account with UPS or FedEx is preferred since they can map your credit card information to your account and thus your credit card numbers do not float around on your packages.
You also need to have your money order/cashier check (in favor of BLS International USA) ready before filling out this form since those details will be required.
Once you complete the order form, it will ask you to select an appointment date and time (in case you selected Walk-in as the application type). One thing to note is that there is a 'First come, first served' rule at BLS office (at least it was there in New York). So even if you have your appointment at 1:00 pm, you should reach there even before the office opens. This will give you enough time to reapply on the same date in case you have some documents missing or they reject your photographs or anything else. There are shops close-by which have notary public & photo services so you can have your documents fixed and then go to BLS again and get a token.
Other documents required
- Notarized color photocopy of proof of valid Visa on Passport/ Green Card/ Permanent Residency Card/ Employee Authorization Card/ I-797 Approval Notice/ I-485 for children, I-20 for applicants on student visa.
- Notarized photocopy of Proof of Residence- For example Driving License/ Utility Bill Gas/Electricity/Water/ Rental lease agreement /Home Mortgage Bill (In case of minor proof of residence of either or both parents is accepted)
- Notarized affidavit for damaged passport. You need to take two witnesses along with their IDs with you to the notary. The applicant should sign as Deponent.
- Notarized '4 point affidavit' - only for applicants of Houston jurisdiction.
- If married but do not have spouse's name in the passport - Notarized photocopy of marriage certificate and Notarized photocopy of the first & last page of spouse's passport.
- Damaged passport along with photocopy of first five & last two pages. Do not send your I-94.
- Duly filled and signed checklist. Make sure you go through the checklist and arrange other documents as needed.
- Prepaid envelopes and labels if applying through or getting it delivered by mail.

ReplyDeleteWhy do we need to send Prepaid envelopes and labels if applying through or getting it delivered by mail.? What does the airway bill number do? This one is created in UPS and applied in the online form.
ReplyDeleteIf you want your new passport to come back to you via postal mail, you need to send them an envelope with a prepaid label which they can use to send your passport back. Airway bill number is the tracking number for your passport. BLS needs it so that they don't have to fill in their records manually. It is more important for you though.

DeleteYes, airway bill number is the same as tracking number. Postal applicants who also choose their passports be returned by post (prepaid delivery: Yes) will have to provide both onwards and return tracking numbers.

DeleteI can tell you for damaged passport renewal, the declaration (item #26, page 3) has to be notarized. That is the only place on the application form which needs to be notarized. Of course, there are other proofs too (like address proof etc as mentioned above) which need notarization.

For the renewal of passport the website mentioned that fill the online application form and 2nd page of the application has to be hand filled. What does that mean? I can only confirm for damaged passport replacement you need 8 copies of nationality verification form. For regular passport renewal, I would think only one copy should be enough but you may send 3 just to be safe.

ReplyDeleteIf you gave Indian address to be printed on passport, make sure you have notarized proofs of that address. You may want to check with BLS too if they allow having Indian address printed on a passport issued in the US. Also, you can submit a new form again - the system doesn't stop you from doing that!
Delete"Continuous" means without a break.. so that will obviously be time since your last visit to India.
Also, I think 4 point affidavit is needed only when we are apply for expired,lost or damaged passport in houston, correct? I am applying for normal renewal, so I did not submit 4 point affidavit..

Category is - re-issue of passport after 10 years through mail service- SFO location. Question is on Online application form. I am done with online application form, I got .pdf file with web file no and I took print out of it. And I got e-mail conformation too. But .pdf file doesn't have date of appointment and time. Is it supposed to show the date of appointment and time. And even e-mail conformation also doesn't have the same. please advise.

DeleteShould we staple the documents. I was looking at the immihelp.com website, they do not mention anything about this in the passport renewal instructions. But in the guidelines for OCI Application they say "Please do not staple your documents. Do not use paper clips either.".

DeleteThere is no set format. Just write a letter on plain paper addressed to the Indian Consulate stating that your signature has changed from what was their on your old passport (attach a snapshot) to the signature done below.

DeleteYou need to create an account with UPS or FedEx, get an envelope from their store and print the shipping label online and paste/pin it on that envelope. If you don't want to print the label online, I think there are envelopes available with the label attached and a tracking number on them, so you can fill the addresses at home, put your account number and UPS/FedEx will bill your card which is attached to your account. Do not enter your credit card details on the label.

Well i am encountering another problem in filling the online application. when i enter my fathers name (i.e. M QASIM MASOODI) the system doesnt accepts it, unless I omit M or mearge it with middle name n it becomes MQASIM. However, in my earlier passport it is as M QASIM MASOODI. So in this application I had only option to use it as MQASIM MASOODI, will there be any problem.

ReplyDeleteAmazing blog buddy. Thanks. This is confusing and its been 1 month I am trying to join the dots. I am confused about Printed shipping label. yes I read all the blogs but still confused. I went to USPS and they gave me tracking number for Shipping and Return envelope. But BLS website says no hand written address accepted. I received an email with To Address. Shall I stick this to my sending envelope. Can I write address on return Envelope or even that has to be pre printed. I think the return envelope must be returned on specific date only. But we are not sure.

DeleteIf BLS website says no handwritten addresses accepted, you should believe them - they are very picky! What they are asking you to do is print shipping labels for both onwards and return shipping. I know this can be done on UPS online - you just have to fill the To and From addresses and it creates the label for you which you can print and stick on the envelopes.

DeleteUPS/Fedex/USPS.. all are different companies to do the same thing. BLS prefers UPS or Fedex. Whichever company you select, you don't have to go to any other company for anything.
DeleteYes, you have to paste those labels on the envelopes. Some envelopes come with a transparent pouch where you can just insert your label (no need to paste it).
Another questions: For Online form Name Address of friends to be US friends or Indian friends. I think National verification form has India friends Address. Also License number is required on Online form when we print and type. So is this Indian license or USA license. Even the declaration is to be notarized on Online form. This i was not aware. Any one any thoughts.

Dear Vivek, when filling the application form by hand, the No. 20 item mentions that if an applicant is in govt. service in India, "No Objection certificate" is needed in original from the Employer. Since i am a visiting scholar at US for 01 year and my employer being Kashmir University, so have i to get the original document form my University.

I would think it is okay if the notarizations are recent and you have ALL documents notarized from one place - either US or India. But I could be wrong too so your best bet is contacting BLS for the same.

I got an email after BLS Order form. I will stick this on my Package and keep the order form inside the package. Is this the Shipping Label or Shipping Label is something else ? I went to USPS site and it also has a shipping label. Are this different or both are required. Sorry for confusing question, but I am still not clear.

ReplyDeleteI also noticed you mention that the declaration on the printer application has to be notarized. I do not see anywhere where this is requested. The BLS instruction set only asks other photocopies etc to be notarized. Can you provide some guidance....

But after filling return info, it processed and charged $28 and i received an email about its confirmation but did not receive any AirwayBill/Tracking number. Now, What is this money taken for? Also, do I have still have to purchase return label again from fedex?.. i am confused
Ok.. That link will give out the Label for "Sending it to the BLS" and "NOT for RETURN"...AFTER SUBMITTING THE BLS FORM. They need to provide full information on their website...

DeleteMy wife is married (obviously, duh) but her current passport has her dad's name as middle name. The form says that "if married but do not have spouse's name in the passport - Notarized photocopy of marriage certificate and Notarized photocopy of the first & last page of spouse's passport is needed". But what if I want to keep my wife's middle name as it is (her dad's name) and do not want it to be changed to my name? Should I select "single" so that there is no confusion for them? There is not a place/box where I can instruct them to keep my wife's middle name as it is if I select "married" and provide all the details. Let me know you have if you have any insight in this topic please.

DeleteWell, according to their checklist and BLS website instructions (Here >> "In case the current passport does not have Spouse’s name, a notarized photocopy of the marriage certificate is mandatory if your status has been filled as married in the application form") NOT sending is not an option since I have to show "Married" in the application. I have been trying to get hold of them over email and phone but no answer yet....I guess at the end if I do not hear back anything soon, I will send the marriage certificate for my wife's passport renewal along with other docs....Thanks again for all your help and instructions.

DeleteI think the reason is online form stays here, while the PP (or the Nationality Verification) forms go to the Indian police for verification (though I don't know if verification does happen, in my case it didn't but if you apply for passport in India, these are the same forms and verification does happen there).

Hi Vivek, I am applying for reissue of a passprt after 10 yrs expiry. My current passport has India's address as permanent address. You said "I gave my US address since I did not want to get into the hassle of procuring my Indian address proofs and getting them notarized". My question is for giving the same Indian address (as in old passport), do I still need to provide Indian address proofs? I don't want to provide US address as it might keep changing depending on my work. I am on H1B.

This page has some details as to how to go to the consulate (of your jurisdiction). Some consulates only accept in person appointments whereas for others, I think a letter on plain paper seeking permission would do. Please contact BLS for more information though and do let us know in case you find out!

And for the second time I came to US in June 2013. So which date I should put for the above question also for the continuous stay abroad which duration I should put. My son came to the US for the first time but prior he visited Singapore for 3 days. So in his case should it be the date he left for US or any foreign country.

DeleteYou submitted your passport in the holiday season where people take long offs. So it could be that they have now got a big line of applications pending which they might be taking time to clear up.
You could go to BLS in person and talk about it. If they give you some kind of reference number, you may go to the Indian consulate too to enquire about your case.

Great blog! With regard to using the FedEx link provided on the BLS order form, is that for shipping docs/application to BLS? Or can that be used to generate shipping label for BLS to return new passport/docs to me?
ReplyDeleteAs one of our readers found out, it is only for shipping to BLS, not for return. I would however suggest you don't use that option and get both onwards and return labels from UPS or Fedex directly (it would be cheaper and can be done online).

DeleteI think if it is a little change in appearance which is natural due to age, it is okay because everyone applies for reissue after 10 years and appearance is bound to change. If it is a major change (or if BLS goes picky), you would need the affidavit. I'm sorry but it is all upto BLS!
When I checked the status online, it says that the application is sent back for the following reason:

a)The apt no is clearly mentioned in the bill. The apartment name isn't mentioned in the bill but I have filled it in the application. Is that considered a different address?
b) The affidavit is to be provided in case of lost passport for more than 6 months isn't it? There was no clause mentioned about an affidavit for passport which was lost. There was a clause mentioning about the affidavit in case of passport which expired for more than 6 months.
Should I get the affidavit in my case where the passport is lost for a month? If so are there any conditions for the witness? Can they be on F1 visa or can they be residents of the US?
Please do Reply as soon as possible. Will the Money order that I sent be returned to me?
1. Your address on the application form needs to be exactly same as on the proof you are submitting.

Now I have the rental agreement with me which contains the same address as in the electric bill, but since it is a lease agreement, it will have apartment name on top. So is that ok? Or can I edit the details in the application? I received the documents. But the money order isn't there. Rather there is a fee payment receipt. Does this mean I have to pay the fee again?
DeleteNo ids go out to BLS other than your own. You need to take the witness (along with his/her id) to the notary who will then sign your affidavit. That's it... no further role of the witness.

Delete1. Page 3 has to be notarized only for damaged/lost passports.
2. If your (or your brother's) name was there on your mom's old passport, then write your/your brother's name in this section. This section is just to have both of you deleted from your mother's passport since now it is required for children to have their own passports. In case your (or your brother's) name wasn't there on her old passport, write N/A.
3. All affidavits have to be notarized.

I'm applying for renewal of my expired passport (it would have been expired for 12 days as of the day of my walk-in). I have a couple of questions:
- Does the Nationality Verification Form need to be submitted for passports expired less than 6 months? If yes, how many copies?
- I'm applying at the Houston consulate. Does the 4-Point Affidavit need to be submitted for passports expired less than 3 years? If yes, how many copies?
The reason I ask is, according the BLS website, the Nationality Verification Form and the 4-Point Affidavit (for Houston applicants) are required ONLY if the passport has been expired for more than 3 years (See link below):

DeleteIn case you have incorrect entries on these pages, you need to sign there, otherwise sign anywhere on the top or the bottom of the pages.
You don't have to pay again. Just send the receipt back (after you keep a copy).

a.Complete Permanent address in India – I live in US now and my address in India has changed from what was on old PP – how do I get this updated as I dont have any bills to provide as proof? All I have is a letter from my Indian bank stating that as my India address. Additionally I can produce a letter from my employer.
b.Date of leaving India – Should this be the first time I ever went out of India or the first time I came to US? I traveled to other countries on work before coming to USA in July 2006 for the first time. After that I have traveled many times between US and India before settling here in 2010.
c.Period of stay abroad – How should this be counted? Should I consider only the longest period I stayed continuously in the US or count all the days\months of all my travels to US(on different visas) and other countries as well?

DeleteIf you have the approval I-797, I don't think you need affidavit for no status. That affidavit is needed only if you had the receipt which said you applied for your visa extension and the case is still pending (as far as what I can understand from their site). Please check with BLS though and let us know!

Since till now I have my father's name in passport, so this time for renewal I have to include spouse name but will it also require to provide affidavit for name change from maiden to married, as I have not yet changed it and will probably not do it as I have to change my name everywhere in all my documents. 1) What should I select for "UPS Packaging Type"? I see UPS Letter, UPS box, UPS tube and other options. My guess is I should select UPS letter, is that correct? Does that mean its an envelope?
2) At the BLS venue, will they collect that envelope and label from me for passport return?

Thanks Vivek for writing such a nice and helpful blog. I came across your blog while researching about passport renewal process from BLS-Washington office. And I must admit your page is one of most informative blogs out here. Anyway, I have one query related to my indian passport renewal. Although, my passport will expire in September, 2015 (almost 1.5 yrs later) but there is only one unused page left. I read somewhere in the BLS website or other page (I can't remeber) that I can only apply for passport renewal as early as 1 year before the current passport expiry date. So, clearly I can't meet this criteria. But I have a plan to visit Canada and Europe in next 5 months. Could you please suggest me something. I am a bit confused! Can I apply for my passport renewal now or I have to wait until I get into 1 year passport expiry timeline.

DeleteIt is true that the BLS page mentions you cannot apply more than an year before the date of passport expiry. But since they have stopped adding additional pages to passport, applying for a fresh passport seems to be the only way out. I would suggest you make a visit to the BLS office with your documents ready. If they agree, you can submit your documents otherwise they would tell you what to do. Please let us know too!

( Your application has been rejected by the consulate because the NOC affidavit is not notarized. Please send a NOC affidavit with the notary stamp. Thank you )
I notarized in last page of the NOC, I have photo copy also.
my question is to whom I need to send the missing document again ? same address ? any thing I need to taken care while sending missing documents ?

DeleteI believe you need to send the documents to BLS again. Same address.
BLS must have returned your documents, if not they might be coming soon. You need to send the affidavit along with them.

ReplyDeleteMy passport application has been sent back due to improper photos. Do i need to fill order form again to send the documents back or can i use old order form with the previous tracking number?
Note: My money order has been not sent back .But they have sent me a receipt for the same.

If the passoprt is gonna expire after 8 months, do we need to send 1 or 3 copies of nationality verification document ? Thanks
